Men are prone to diseases, especially in the reproductive organs. For example, prostatitis or inflammatory diseases of the penis are common. The causes are complicated, so they need to be carefully studied and treated. There are many types of penis diseases, inflammation is common, and of course there are other symptoms. It will affect physical health and also affect the sex life, so what is the reason for slow blood flow in the penis?

Penile blood flow detection: Penile vascular disease is an important cause of organic erectile dysfunction, namely, arterial blood supply obstruction and venous blockage mechanism disorder.

Vasoactive drug-induced erection test: Currently, 30-60 mg of poppyrine, 1-2 mg of phentolamine, or 20 μg of prostaglandin E are mostly used, either alone or in combination. The drug is injected into the cavernous body, and the penis can achieve a hard erection that lasts for more than 30 minutes, indicating that there is no significant vascular lesion, but there is still a possibility of false negative. The reliability is higher when sexual stimulation is used after the injection. Complications such as ecchymosis, hematoma, and abnormal penile erection may occur.

Penile Doppler ultrasound monitoring: Measure the ratio of penile artery blood pressure to brachial artery blood pressure (PBI). A value less than 0.6 indicates penile artery blood supply obstruction. The difference between the two absolute systolic pressure values ​​should not exceed 4 kPa (30 mmHg).

Penile flow index (PFI): Penile blood flow index is calculated by measuring the acceleration of radial artery, dorsal penile artery and cavernous body artery with Doppler ultrasound probe. PFI < 6 indicates normal penile blood supply. Recording of penile arterial blood flow pulse volume: The normal penile blood flow pulse volume waveform rises rapidly to the peak and then slowly decreases to form a double wave pulse notch. A round peak or a slow decrease and disappearance of the double wave pulse notch indicates vascular lesions.

Color Doppler ultrasound examination: It can detect the cavernous body structure, blood vessel diameter, blood flow velocity and vasodilation function. It can dynamically detect the hemodynamic changes of the penile artery and vein during erection, the cavernous body resistance index, etc. It is one of the most valuable non-destructive examination methods for screening vascular erectile dysfunction.

Dynamic infusion cavernosometry & cavernosography (DICC): usually monitor the induction flow (IF), the minimum flow rate to maintain an erection (MF), and the pressure loss (PL) gradient of the cavernous body within 30 seconds after stopping the perfusion. The larger the MF and PL values ​​are, the more likely there is venous leakage erectile dysfunction. Normal PL should be <3.3kPa (25mmHg) within 30 seconds, MF should be <20-40ml/min, and IF should be 80-120ml/min. Cavernography is a procedure that injects contrast agents to observe whether there is abnormal venous reflux during erection. Common abnormal reflux includes: leakage from the deep dorsal vein of the penis to the prostatic plexus and internal pudendal vein, leakage from the cavernous vein to the prostatic plexus and internal pudendal vein, and leakage between the cavernous body of the penis and the cavernous body of the urethra.

Internal pudendal artery angiography: For patients with suspected penile artery blood supply obstruction, bilateral internal pudendal artery angiography should be performed through the femoral artery before penile artery reconstruction to observe the lesions of the dorsal penile artery and cavernous body artery on both sides.
