Inflammatory changes are the most common cause of abnormalities or pain, such as posterior urethritis, fossa urethritis, prostatitis, seminal vesiculitis, vas deferens, ejaculatory ductitis, etc. Inflammation causes local congestion and edema, accompanied by rhythmic muscle contraction during ejaculation and cramping pain. Infection can also cause urethral stenosis, resulting in poor semen discharge and pain during ejaculation.

If the skin of the penis is allergic to condoms or contraceptive creams, it can cause allergic dermatitis and cause pain during intercourse. This can be avoided by stopping or switching to other contraceptive methods. If there are ulcers on the coronal sulcus and neck of the penis, scars will be left after healing. When the penis is erect and twitching during intercourse, pulling pain will occur. In this case, surgery is required to remove the scar tissue.
