What are the symptoms of benign gastric tumors?

Gastric adenoma (adenoma of stomach) can occur at any age, but is more common in men over 40 years old. The incidence is higher in patients with atrophic gastritis, gastric achlorhydria and pernicious anemia. It often occurs in the gastric sinus, and the base often has a pedicle, which can exist alone or in multiples. Adenoma is polyp-like when observed with the naked eye, so it is also called adenomatous polyp. The disease is asymptomatic in the early stage. When there are complications, there may be upper abdominal discomfort, dull pain, nausea, vomiting and bleeding. Pyloric pedunculated adenomas can enter the duodenum through the pyloric duct, causing intermittent pyloric obstruction, and even gastroduodenal intussusception. Patients may have anemia and a positive stool occult blood test. Diagnosis mainly relies on X-ray barium meal examination and gastroscopy. Gastroscopy can not only diagnose the location, shape, size and number of adenomas, but also determine whether there is malignancy through biopsy.

Benign gastric tumors are rare and usually do not include gastric polyps. Benign gastric tumors account for about 3% to 5% of gastric tumors and can be divided into two categories: epithelial tumors (adenomas, papillomas) and mesenchymal tumors (leiomyoma, fibroma, neurofibroma, lipoma, and hemangioma). The former accounts for about 3/4 of benign gastric tumors, and the latter accounts for about 1/4. The most common clinical types are adenomas and leiomyomas. Benign gastric tumors rarely cause symptoms. Some symptoms may occur only after complications or malignant changes occur due to the large growth of the tumor. The symptoms vary: discomfort and pain in the stomach area, similar to symptoms of gastritis or ulcer disease, sometimes chronic small amounts of bleeding, and occasionally heavy bleeding. When the tumor is located near the anus or pylorus, it may cause obstructive symptoms. Therefore, benign gastric tumors are easily overlooked.

Tumors are usually solitary, but occasionally multiple. The most common location is in the gastric body, followed by the gastric sinus, followed by the gastric fundus, pylorus, and pyloric ventricle. They are generally round or oval, hard, without a true capsule, with a smooth surface, and may be lobed. Most are pedunculated. Small tumors are confined to the gastric wall, and when they grow larger, they may protrude into the gastric cavity, or protrude under the serosa, or protrude inward or outward and appear dumb-bell-shaped. Sometimes they protrude from the serosa surface with one end free in the peritoneal cavity.

After the diagnosis of benign gastric tumor, those who can undergo gastroscopy can undergo surgery directly, depending on the equipment of each hospital. The patient should fast for 12 hours before the operation, and routinely check for cardiopulmonary complications. Check whether the Nd:YAG laser is working properly and whether the quartz laser knife is broken.
