What is bladder hernia?

The bladder is an extraperitoneal organ. The space between the pubic symphysis and the retropubic space is also called the anterior vesical space. The space is filled with loose connective tissue and the pubic veins. The space behind the bladder is adjacent to the rectum in men and the uterus and the anterior wall of the vagina in women. In men, there are tissues such as the seminal vesicle and vas deferens. The bladder is covered with peritoneum and adjacent to the intestinal loops. If the intestinal loops and peritoneum protrude into the retropubic space and the retrovesical space, it is called a superior vesical internal hernia. An internal hernia occurring in the retropubic space is also called anterior superior vesical internal hernia, and an internal hernia occurring in the retrovesical space is also called posterior superior vesical internal hernia.

Causes

The cause of the disease is unknown, but the existence of the retropubic space and retrovesical space is the anatomical basis of this disease.

Clinical manifestations

1. Intestinal obstruction symptoms: Because the hernial sac is small, the herniated intestine is often a single intestinal loop, and there are usually no obvious clinical symptoms. When the herniated intestinal loop becomes incarcerated, symptoms of intestinal obstruction such as lower abdominal pain, bloating, and vomiting will occur.

2. Bladder irritation symptoms: When intestinal incarceration occurs, the intestinal loops swell and compress the bottom of the bladder, which can cause bladder irritation symptoms such as frequent urination and urgency.

treatment

The disease is mainly treated with surgery. Generally, a midline or paramedian incision is made in the lower abdomen. During the operation, a urinary catheter is placed to empty the bladder, the herniated intestine is repositioned, the hernial sac is cut open, the neck of the hernial sac is sutured with non-absorbable sutures, and the defect on the bladder is closed to prevent recurrence after surgery.
