Gynecomastia is a common clinical disease. It is generally considered abnormal to touch breast tissue in men, except for three cases: transient gynecomastia in newborns, breast enlargement during puberty, and occasional gynecomastia in older men.

Male breast development is caused by increased estrogen secretion or decreased androgen/estrogen ratio. Excessive estrogen is the main cause of male breast development. Giving men exogenous estrogen preparations, such as estrogen treatment for prostate cancer patients, long-term use of estrogen by transgender men, and excessive estrogen secretion by adrenal or testicular tumors can all lead to breast hyperplasia.

Male breast tissue can be palpated on one or both sides, presenting as disc-shaped nodules or diffuse enlargement, sometimes accompanied by enlargement of the nipple and areola. Local pain or tenderness may be felt, and a small amount of white secretions may be seen leaking out when the nipple is squeezed in a few patients. Pathological male breast development caused by organic diseases should also have clinical manifestations of the primary disease.

treatment

1. Dihydrotestosterone enanthate 200 mg, intramuscularly once every 3 to 4 weeks.

2. Tamoxifen (tamoxifen) can bind to the estrogen receptors in the target tissue and block the effect of estrogen.

3. The mechanism of action of clomiphene is similar to that of tamoxifen (tamoxifen).

4. Testosterone lactone inhibits aromatase, blocking the conversion of testosterone into E2 in the periphery.

5. Mammoplasty Due to the irreversibility of long-term delayed gynecomastia, mammoplasty is still an important means of treating this disease. Generally, the subareolar breast tissue is removed through the peri-areolar approach.
