What department should men go to if they can’t hold their urine?

In daily life, men have difficulty holding back urine, but many people do not know much about it, and are not very clear about its causes and treatment methods. In fact, the inability to hold back urine is what is clinically called urinary incontinence. There are clear clinical classifications of urinary incontinence, and different types of urinary incontinence have different manifestations. In this case, which department should I go to for treatment?

Overflow incontinence is caused by urinary retention due to severe mechanical or functional obstruction of the lower urinary tract. When the intra-bladder pressure rises to a certain level and exceeds the urethral resistance, urine drips continuously from the urethra.

Reflex incontinence is caused by complete upper motor neuron disease, in which urination depends on spinal reflexes and the patient urinates intermittently and involuntarily.

Stress incontinence is urine leakage from the urethra when abdominal pressure increases. The causes of this type of incontinence are complex.

Non-resistance urinary incontinence is due to the complete loss of urethral resistance, the bladder cannot store urine, and the patient's urine flows out of the urethra when standing.

Urge urinary incontinence can be caused by strong local stimulation such as partial superior motor neuron disease or acute cystitis.

Treatment

1. Insufficient sphincter function: This type of patient has residual urine. The principle of treatment is to increase the resistance of the urethra by drugs or surgery. Patients with non-resistance urinary incontinence can implant an artificial urinary sphincter device or urethra lengthening surgery.

2. Large amounts of residual urine can cause stress incontinence or overflow incontinence. The principle of treatment for this type of incontinence is to use surgery (bladder neck or external urethral sphincter incision) to reduce urethral resistance and reduce residual urine.

3. Detrusor hyperreflexia or unstable bladder can cause urgency or reflex incontinence, and sometimes cough urgency incontinence. The treatment principle is to inhibit the uninhibited contraction of the bladder with drugs, sacral nerve block, sacral nerve surgery or bladder nerve stripping.

The above are the types and treatments of urinary incontinence. In fact, the main cause of urinary incontinence is the damage of the bladder and nervous system. In view of the above causes and treatments, when urinary incontinence occurs, you can go to the urology, surgery and neurology department for treatment. According to the different degrees of urinary incontinence, patients can choose according to their own circumstances.
