Many people may not be very familiar with the condition of prostate duct obstruction, but once it occurs, it will cause great harm to the body. In the end, male friends will no longer be able to have sex and will have sexual dysfunction, which will affect future fertility. Therefore, relevant examinations should be done as soon as possible, such as semen analysis. Prostate duct obstruction 1. Semen analysis Through more than 3 times of semen analysis and centrifugal sedimentation examination, confirm that there is no sperm. At the same time, pay attention to the semen volume, sperm count, sperm motility, semen pH, viscosity, etc. 2. Biochemical test of serum Seminal plasma is composed of testicular fluid, epididymal fluid, prostatic fluid and paraurethral gland secretions. The determination of seminal plasma biochemical components can understand the function of accessory glands and provide a basis for differential diagnosis of azoospermia and location of vas deferens obstruction. It includes the examination of fructose, neutral α-glucosidase, prostate D synthase and inhibin B. 3. Ultrasound Transrectal ultrasound can measure the axial and sagittal diameters of the vas deferens, seminal vesicles, ejaculatory ducts and prostate, and observe their internal structures. It is particularly useful for identifying congenital developmental abnormalities and ejaculatory duct obstruction. Doppler ultrasound can locate testicular puncture and sperm aspiration based on the distribution of blood vessels in the testicle. 4. Genetic testing In patients with azoospermia, there are chromosomal abnormalities, and chromosome testing is routinely performed on patients with azoospermia. 5. Vasovesicle angiography Vasectomy is the most commonly used method to confirm vas deferens obstruction. Testicular biopsy should be performed before vasectomy. Only those with basically normal testicular spermatogenesis can undergo vasectomy. If the testicles have no spermatogenesis function, vasectomy is meaningless. 6. Scrotal Exploration Scrotal exploration is an important means of diagnosing vas deferens obstruction. The order of exploration is testis, epididymis, and vas deferens. During the exploration, testicular biopsy can be performed. It is best to perform exploration and surgery at the same time to avoid secondary surgery. 7. Testicular biopsy Testicular biopsy can be used to differentiate between obstructive and non-obstructive azoospermia and to evaluate testicular spermatogenesis. Sperm can be retrieved from the testicles, and intracytoplasmic sperm injection (ICSI) can be performed for patients with non-obstructive azoospermia and obstructive azoospermia that cannot be surgically recanalized. |
