Treatment of testicular tuberculosis

Testicular tuberculosis is caused by diseases such as vas deferens obstruction or varicose veins, which can lead to the destruction of the patient's accessory glands. In severe cases, it can also affect the male's fertility. Therefore, when testicular tuberculosis occurs, streptomycin or rifampicin can be used for treatment. 1. Although male internal genital tuberculosis can be cured, it often causes blockage of the vas deferens and damage to the accessory gonads, making fertility impossible to guarantee. This shows the importance of early and thorough treatment of this disease.

2. If there are primary lesions such as pulmonary tuberculosis or renal tuberculosis, they should be actively treated at the same time.

3. Western medicine often uses intramuscular injection of streptomycin and oral administration of remifampicin for this disease. If the treatment is ineffective, anti-tuberculosis drugs such as rifampicin, kanamycin, pyrazinamide, and puromycin can be used.

4. Patients with epididymal tuberculosis whose nodules are larger than 0.5-1 cm in diameter and have formed cold pustules or sinus ducts should undergo epididymotomy and vas deferens removal if the disease fails to heal. However, the testicles should be retained as much as possible.

5. Pay attention to rest, have sexual intercourse in moderation, avoid excessive fatigue, and usually support the scrotum with a cloth belt or scrotal support.

6. Strengthen nutrition, mainly focus on cleansing and nourishing. It is advisable to eat chicken, fish, eggs, cod, turtle, mussels, white fungus, scallions, sea cucumbers, etc. Eat more fresh vegetables and fruits, and avoid spicy and irritating foods.

Tuberculous epididymitis is epididymal tuberculosis, which is generally easy to diagnose. However, in the early stage and acute onset of the disease, it should be distinguished from other epididymal diseases and scrotal diseases. Diseases that are easily confused with epididymal tuberculosis include chronic epididymitis and scrotal filariasis.

Early epididymal tuberculosis should be differentiated from chronic epididymitis. Chronic epididymitis is more painful and often has a history of acute and recurrent attacks. The epididymal tumor is not as hard or large as tuberculosis, and rarely forms localized nodules. No sinus tracts are formed, and there are no skin adhesions or beaded changes in the vas deferens.

Gonococcal epididymitis is characterized by a history of gonococcal infection, with an acute onset and local redness, swelling, heat, and pain. There is purulent secretion in the urethra, in which intracellular Gram-negative diplococci can be found. The urethral secretions can be tested for the specific amino acid sequence of gonococci by polymerase chain reaction (PCR).

Chlamydia-induced epididymitis can also cause similar acute epididymitis to gonococcal epididymitis. Patients have a history of non-gonococcal urethritis, and the urethral secretions are mostly thin and white.

Scrotal filariasis can sometimes be confused with tuberculous epididymitis. The infiltration and nodules caused by filariasis are in the spermatic cord near the epididymis and can be separated from the epididymis. The nodules of filariasis often undergo significant changes in a short period of time, while those of tuberculosis change very slowly. Filariasis is regional, and patients may have both rubber swelling and chylous hydrocele.

A normal epididymis is sometimes misdiagnosed as epididymal tuberculosis. It is normal for the epididymal head and tail to be slightly larger or harder. If there is no infiltration or nodules, the diagnosis cannot be confirmed and can be observed during follow-up.
