1. Vacuum constriction device

The vacuum constriction device can be used for erectile dysfunction caused by any reason and is a second-line method for treating ED. However, the hemodynamics that cause erection are different from those of normal erection, and there is no active relaxation of the corpus cavernosum and smooth muscle. Animal experiments have shown that arterial blood flow does not increase after use, but venous return is significantly reduced, and the corpus cavernosum and penile skin are filled with blood, resulting in penile enlargement. This method is suitable for patients who have failed to respond to PDE5 inhibitor treatment or cannot tolerate drug treatment, especially for elderly patients who occasionally have sex. Adverse reactions include penile pain, numbness, delayed ejaculation, etc., and the patient should be informed. The negative pressure erection time should not exceed 30 minutes. Contraindications include patients with spontaneous priapism, intermittent priapism, and severe penile deformities.

2. Cavernous body injection therapy

Intracavernous drug injection is to inject vasodilators into the corpus cavernosum of the penis to make it congested and achieve penile erection. Currently, the most commonly used drugs for intracavernous drug injection in the treatment of erectile dysfunction are popliteal alkaloids, phentolamine and prostaglandin E1. This method is effective and fast. With the widespread use of oral drugs, this method is less and less used in clinical practice because it is an invasive operation and has side effects such as pain, bleeding, abnormal penile erection and penile fibrosis.

3. Surgical treatment

With the advent of new drugs and the increased understanding of the pathogenesis of erectile dysfunction, surgical treatment has gradually decreased, but there are still some patients with erectile dysfunction who need surgery to solve the problem, generally those who have failed to respond to other treatments. Surgical treatments include prosthesis implantation, arterial vascular reconstruction and venous ligation.
