Treatment principles Emphasize early diagnosis, early treatment, regular treatment course, and adequate dosage. Regular clinical and laboratory follow-up after treatment. Sexual partners should be examined and treated together. Early syphilis can be clinically cured and eliminate infectiousness through thorough treatment. Treatment of late syphilis can eliminate tissue inflammation, but the damaged tissue is difficult to repair. Penicillin, such as aqueous penicillin, procaine penicillin, and benzathine penicillin, are the first choice drugs for different stages of syphilis. Tetracycline, erythromycin, etc. can be selected for those who are allergic to penicillin. Some patients may have a Jasper-Herzal reaction at the beginning of penicillin treatment, which can be prevented by starting with a small dose or using other drugs. The serum should be checked every 3 months in the first year after syphilis treatment, and every 6 months thereafter for a total of 3 years. Neurosyphilis and cardiovascular syphilis should be followed up throughout life. 2. Early syphilis (including primary, secondary syphilis and early latent syphilis) (1) Penicillin therapy: Penicillin G benzathine (long-acting penicillin) is injected intramuscularly into both buttocks once a week for 2-3 times. Penicillin G procaine is injected intramuscularly for 10-15 consecutive days, with a total dose of 8 million units to 12 million units. (2) For those who are allergic to penicillin, take tetracycline hydrochloride orally for 15 consecutive days. Take doxycycline orally for 15 consecutive days. 3. Late syphilis (including tertiary cutaneous, mucosal, and bone syphilis, and late latent syphilis) and secondary recurrent syphilis (1) Penicillin G benzathine, once a week, intramuscularly, for a total of 3 times. Procaine penicillin G, intramuscularly, for 20 consecutive days. The treatment can be repeated once after an interval of 2 weeks. (2) For those who are allergic to penicillin, take tetracycline hydrochloride orally for 30 days. Take doxycycline orally for 30 days. 4. Neurosyphilis The patient should be hospitalized for treatment. To avoid the occurrence of Gibson-Hertz reaction during treatment, oral prednisone should be taken one day before the penicillin injection, once a day, for 3 consecutive days. (1) Aqueous penicillin G intravenously for 14 days. (2) Procaine penicillin G is injected intramuscularly and probenecid is taken orally for 10 to 14 days. After the above treatment, penicillin G was continued, once a week, intramuscularly, for 3 consecutive weeks. |
