1. How to treat testicular dislocation?

Generally speaking, when the scrotum is hit, the testicles reflexively shrink upwards. Therefore, if the hit is not serious, as an emergency first aid measure on the spot, the injured person should be forced to stand up and jump vigorously so that the shrunken testicles can quickly return to their original position. Otherwise, if the testicles are squeezed upwards due to strong external force and are in an abnormal position for a long time, the blood vessels will be twisted, the testicles will be compressed, the blood supply to the testicles will be interrupted, and the ischemia time will be too long, which may lead to necrosis of the testicular tissue itself.

2. Symptoms of testicular malposition

Due to the blunt force of the perineum squeezing the testicles, the testicles cannot return to their original normal position after displacement. The location of the testicle after dislocation depends on the size, direction, nature of the force and the weak links of the local anatomy. The testicles are often dislocated in the inguinal canal and perineum. The symptoms are severe pain in the groin or perineum after trauma. During examination, the scrotum is found to be empty, and painful lumps can be felt in the corresponding parts outside the scrotum.

Testicular contusion

It is often caused by direct kicking, falling from a height, or squeezing the testicles when riding. Severe pain often radiates to the lower abdomen and ipsilateral waist, accompanied by nausea and vomiting. In severe cases, it can cause painful shock. During examination, scrotal enlargement, subcutaneous hemorrhage, testicular swelling and obvious tenderness can be seen. Due to the restriction of the solid white membrane of the testicle, the internal pressure increases and the hard testicles can be touched.

Ruptured testicle

Generally, it is caused by open injury, and testicular tissue is often exposed. Testicular rupture caused by blunt injury is rare. The main symptoms are severe pain after injury, and even nausea, vomiting, and fainting. During examination, scrotal ecchymosis, testicular enlargement, unclear testicular outline, and obvious tenderness are found.

Testicular torsion

Testicular torsion may be related to anatomical malformations, such as excessive enlargement of the sac vaginalis, incomplete testicular descent, etc. The part below the torsion first undergoes congestion and hemorrhagic infarction. Due to the spasm and tension of the levator testis muscle, the testicle is pulled back. The clinical manifestations are local swelling, obvious tenderness, and the touch of a testicular mass, which is helpful for diagnosis.
