Testicular ptosis is a normal phenomenon. It is an automatic protective function of the body to protect sperm. When the weather is hot, the temperature of the testicles rises, which will cause sperm to die, so the scrotum sags to dissipate heat. On the contrary, when the weather is cold, the scrotum automatically shrinks to keep the testicles at a certain temperature. The scrotal skin is very loose and its changes are relatively large. For example, it will shrink when you are nervous or stimulated by cold. If there is no other discomfort, don't worry. If the testicle volume is larger than before, consider the possibility of hydrocele and other conditions. If necessary, you can go to the hospital for examination and diagnosis. If the testicles on both sides of the scrotum are equal in size, round, smooth and symmetrical, it is considered that the scrotum is just loose and there should be no problem.

The function of the scrotum, in addition to being an external barrier, is also a unique temperature-regulating function from a reproductive perspective, protecting the testicles inside at a suitable temperature. When in a cold environment, the scrotal skin will shrink and thicken, causing the scrotal lashes to be tense and the good surface area to shrink. This state can protect the temperature of the testicles in the scrotum from decreasing; on the contrary, in a warmer environment, the scrotal skin will naturally relax, and the support and covering of the scrotum, the suspension of the spermatic cord and the levator testis all play a mechanical protective role on the testicles, which can buffer the vibration of the testicles when walking and exercising, and are of certain significance in preventing trauma.

The scrotal skin is very sensitive to the external temperature. It has no subcutaneous fat but abundant sweat glands, which help to dissipate heat. When swimming, the water temperature is much lower than body temperature, which stimulates the contraction of the smooth muscle of the fascia and the levator testis muscle, raising the position of the testicles. The scrotal skin shrinks into dense wrinkles and retracts to the perineum, preventing heat dissipation and helping to keep warm. The same phenomenon occurs when the weather is cold.

On the contrary, when the external temperature rises, the smooth muscle and levator testis relax, the testicles descend and leave the body, the scrotal skin relaxes, and the heat dissipation area is increased, which is conducive to local heat dissipation. In addition, the arteries in the spermatic cord are entangled in the bundles of parallel venous clusters. The blood flows in the two vascular systems in opposite directions, separated only by a thin vascular wall, forming a countercurrent exchange system. The venous blood continuously takes away the heat from the arterial blood in the abdominal cavity. As a result, the blood temperature of the testicular artery can be 5.2°C lower than that of the abdominal aorta.
