Free PSA refers to the part of PSA that is free in the plasma and not bound, expressed as fPSA; total serum PSA is expressed as tPSA. The fPSA concentration is lower in cancer patients than in benign hyperplasia patients. This difference is applied clinically to screen out early prostate cancer from patients with benign prostate hyperplasia. Currently, the fPSA/tPSA ratio is widely used to assist in distinguishing prostate cancer from benign hyperplasia. The reference value is 0.16, that is, if the ratio is <0.16, the possibility of prostate cancer is high. A low fPSA percentage indicates a higher likelihood of prostate cancer. 2. Serum PSA is a specific marker for prostate cancer It is very meaningful for the diagnosis of early asymptomatic prostate cancer. Under normal circumstances, PSA is a serine protease secreted by prostate epithelial cells and is a glycoprotein that is directly secreted into the prostate duct system. Its normal function is to assist in the hydrolysis and liquefaction of semen clots, which is related to male fertility. There is a blood-epithelial barrier around the normal prostate duct system, which prevents PSA produced by the prostate epithelium from directly entering the blood, thereby maintaining a low concentration of PSA in the blood. It is generally believed that a serum PSA less than 4.0 ng/ml is normal, and a PSA greater than 10ng/ml increases the risk of prostate cancer.
