What are the methods for diagnosing prostatitis?

1. Prostate and rectal indication

Transanal palpation of the prostate is an important means of clinical diagnosis of prostatitis, and can obtain important information that is indispensable for the diagnosis and treatment of prostatitis. Direct palpation of the prostate has many advantages. This method is simple, direct, convenient, and economical. It can clearly determine the size, texture or hardness, temperature, whether the central groove has become shallow or disappeared, whether the surface is smooth, whether there are nodules, whether there is a sense of fluctuation, whether there is tenderness, the degree of pain, and the congestion of the surface.

2. Examination of urine and urethral secretions

The main purpose is to check the urine routine and the number of white blood cells in the secretions to determine whether the infection comes from the urethra or bladder. The urethral secretions can also be cultured to find pathogenic microorganisms, thereby inferring the impact on the prostate. The traditional four-cup urine test is considered to be the basic method for locating and culturing urinary tract bacteria.

3. Routine examination of prostatic fluid

Prostatic fluid cytology is a common method of laboratory diagnosis. It mainly determines whether the prostate is inflamed and the degree of inflammation by checking the number of lecithin bodies, red blood cells, white blood cells or pus cells. Samples are taken by massaging the prostate. If the number of lecithin bodies in the prostatic fluid is more than +++/HP (per high-power field microscope), the number of white blood cells is less than 10/HP, there are no pus cells, and there are no or occasional cells, it means that the prostate is not infected or prostatitis does not exist.

IV. Prostate pathogen culture and drug sensitivity test

Identifying the pathogen and conducting a drug sensitivity test is the most ideal diagnostic measure. The test result is one of the important factors related to the treatment effect, because it is an important basis for doctors to choose antibiotics. However, most of the bacterial culture methods commonly used in clinical practice are not very accurate, and the results are often specious. The positive bacteria found are not necessarily the pathogens of prostatitis, but may be the result of urethral contamination. Therefore, this factor should be considered when analyzing the results.
