Sperm examination includes the shape, total number and function of sperm, as well as the cell biology, medical immunology, and molecular biology of sperm. Sperm motility tests such as sperm survival rate, sperm vitality, and sperm motility are key items in sperm testing. When the structure and function of the testicles, ejaculatory ducts, and associated sex hormones are damaged or mutated, they can affect the quality of semen.

Normal sperm test result report: When the result is normal, the hospital outpatient examination form usually describes: a text description of normal sperm appearance and normal values ​​of sperm and sperm quality analysis. (Sperm testing is mainly based on numerical results, and also has a simple text description of sperm appearance characteristics)

1. Physics test

(1) Appearance: Grayish white or milky white, not completely transparent. Normal sperm can vaporize on its own after being left for a period of time, and appear transparent and milky white. The semen of men who have not ejaculated for a long time can appear light yellow.

(2) Quantity: According to the reference standard, the amount of semen discharged at one time is 1.5~6.8ml.

(3) Condensation and vaporization: Normal semen is in a jelly state after being ejaculated, which means that the semen is solidified. The time required for semen to change from a jelly state to a flowing state is called the sperm liquefaction time.

Reference: Men's ejaculated semen condenses immediately at night and begins to vaporize after 15 minutes. The vaporization time is less than 60 minutes.

(4) Viscosity: The length of the metal wire is less than 2 cm, and the sample is water-like, producing discontinuous droplets. Increased viscosity affects the test of sperm motility, sperm concentration, male sperm surface antigens and biochemical markers.

(5) pH value: Reference range: pH 7.2~8.0 (average pH value is 7.8).

2. Sperm organic chemistry test

(1) Glucose specificity of refined plasma

Reference range: Resorcinol method, 9.11~17.67mmol/L; indole colorimetric method, one male ejaculation ≥13μmol/L.

(2) Specificity of neutralized α-glucosidase in refined plasma

Reference: Colorimetric method, a man's ejaculation is ≥42.6mU/mL.

(3) Specificity of lactate dehydrogenase isozyme-X (LD-x) in refined plasma

Reference: Flocculant electrophoresis principle method, relative specificity ≥ 42.6%, positive specificity (1430 ± 940) U/L.

(4) Specificity of ACP

Reference: ammonium phenylene disodium sulfate colorimetric method, 48.8~208.6U/mL.

3. Optical microscopic examination

(1) Male sperm counting

Reference range: male sperm count (15-213) periodontal 109/L; male sperm count in one ejaculation (39-802) periodontal 106/L.

(2) Sperm morphology

Sperm morphology test is to understand the proportion of normal sperm and sperm with genetic mutations within the physiological and pathological ranges, which is a key indicator of male reproductive function. Through the microscope, sperm morphology can be observed and the percentage of normal and various sperm abnormalities in 200 sperm can be calculated.

Under a microscope, normal sperm is frog-shaped, consisting of a head, body (neck, mid-region), and tail. The back of the head is oval, and the sides are flat and pear-shaped; the body has a straight and standard outline; and the tail is long and thin.

Reference range: Normal sperm 4%-44% (abnormal sperm should be less than 20%, if it exceeds 20% it is abnormal).

(3) Sperm survival rate

Sperm motility includes sperm survival rate, sperm vitality, and sperm motility. Sperm survival rate is the percentage of sperm survival after a certain period of time. Under normal circumstances, the sperm survival rate should be above 75% 30 minutes to one hour after ejaculation, and should be >20% after 6 hours.

Reference range: survival rate ≥ 75%.

(4) Sperm motility

Sperm motility refers to the proportion of active sperm, which refers to the proportion of active sperm in the total number of sperm observed directly under a microscope.

Reference: Under normal circumstances, sperm motility should be 60% to 65% or above 30 minutes to 1 hour after ejaculation.

(5) Male sperm motility

Male sperm motility indicates the level of male sperm activity. Vaporized sperm is placed on a glass slide, covered with a glass slide, and 5 to 10 lines of sight or at least 200 male sperm are observed under a microscope to observe and record the activation status of male sperm. Sperm motility is classified into 4 levels according to the WHO's strong recommendation:

a. Grade III (linear movement): The sperm activity of the male is excellent, showing a rapid and cheerful linear forward movement.

b. Grade II (slow movement): Male sperm can move, showing slow linear or non-linear forward movement.

Class cI (stationary exercise level): Male sperm activity is poor, standing still or moving in a rotating manner, not forward exercise.

dO grade (immobile): Male sperm is immobile.

Reference example: Under normal circumstances, 30 minutes to 1 hour after ejaculation, the proportion of grade III sperm should reach more than 50%.

1. Please eat light food, avoid drinking alcohol and fatigue the day before the routine physical examination. Please fast and avoid food on the day of the routine physical examination.
