White spots on the glans may be caused by glansitis. Inflammation of the glans mucosa is called glansitis, which can be caused by various pathogens, local irritation and other factors. Usually, glansitis refers to inflammation of the inner plate of the foreskin and the glans penis. A lipid substance secreted in the normal foreskin cavity can accumulate into smegma to irritate the foreskin and glans penis when the foreskin is too long or phimosis occurs, causing glansitis.

Balanitis can be clinically divided into the following types:

1. Acute superficial balanoposthitis: At the onset, local flushing, redness and swelling of the penis skin, and burning and itching sensation in the glans. When the foreskin is turned back, the inner surface of the foreskin and the glans can be seen to be congested and eroded, with exudate and even bleeding. After secondary infection, small ulcers can be seen, with foul-smelling milky white purulent secretions. If it rubs against underwear, it will be painful, and the patient often has difficulty in moving. It may be accompanied by enlargement and tenderness of the inguinal lymph nodes.

2. Candida albicans balanitis: Erythema can be seen on the foreskin and glans, with a smooth surface and small blisters. The edges of the erythema are clear, and there are erosions and exudates during acute attacks.

3. Trichomonas balanoposthitis: papules and erythema appear on the glans, gradually expanding with clear edges. Small blisters the size of a needle can be seen on the erythema, and finally an erosive surface is formed.

4. Annular ulcerated balanoposthitis: Erythema can be seen on the glans and foreskin, which gradually expands and becomes ring-shaped, and may form superficial ulcers.
