Symptoms and treatment principles of bacterial prostatitis

What are the symptoms of prostatitis?

1. Urinary discomfort: patients often experience frequent urination and urethral burning pain, which can radiate to the glans penis. In the morning, there is mucus, sticky threads and pus secretion from the urethra, and the urine is turbid or white after urination. In severe cases, there may be hematuria at the end of urination, difficulty urinating or urine retention.

2. Sexual dysfunction: Patients may experience hypersexuality in the early stages, and in the later stages may suffer from decreased or absent libido, painful ejaculation, hematospermia, premature ejaculation, impotence, spermatorrhea and infertility.

3. Local symptoms: The patient will feel abnormality, pressure or fullness in the posterior urethra, perineum and anus, especially when squatting or defecating.

4. Radiating pain: When inflammation occurs, the tension inside the gland increases, which can stimulate the sympathetic nerves and cause metastatic low back pain. The pain can radiate to the penis, testicles, scrotum, groin, perineum, lower abdomen, thighs, buttocks, rectum, etc.

5. Abnormal symptoms: Patients with chronic prostatitis may experience fatigue, dizziness, insomnia and depression. Some may also experience various special sensations, such as feeling heavy-headed, coldness in the lower abdomen, fever in the head and face, etc.

The purpose of treatment is mainly to relieve symptoms and improve quality of life, and the evaluation of treatment efficacy should be based mainly on symptom improvement.

(1) General treatment: quit drinking alcohol and avoid spicy food; avoid holding urine and sitting for long periods of time, keep warm, and increase physical exercise.

(2) Drugs commonly used in drug treatment include antibiotics, beta-receptor blockers, herbal preparations and non-steroidal anti-inflammatory analgesics. Other drugs are also effective in relieving symptoms.

① Antibiotics: Available antibiotics include fluoroquinolones (such as ciprofloxacin, levofloxacin, lomefloxacin and moxifloxacin, etc.), tetracyclines (such as minocycline, etc.) and sulfonamides (such as co-trimoxazole).

② - Receptor blockers - Receptor blockers can relax the smooth muscles of the prostate and bladder, improve lower urinary tract symptoms, and thus become the basic drugs for the treatment of prostatitis. - Receptor blockers mainly include: doxazosin, naftopidil, tamsulosin and terazosin, etc. - The course of treatment with receptor blockers should be more than 12 weeks.

③ Plant preparations mainly refer to pollen preparations and plant extracts, including: Pusita, Sabah palm and its extracts, etc. The adverse reactions are relatively small.

④M-receptor blockers (such as tolterodine) can be used to treat prostatitis patients with overactive bladder (OAB) symptoms such as urgency, frequency and nocturia but without urinary tract obstruction.

⑤ Antidepressants and anti-anxiety drugs For patients with chronic prostatitis who also suffer from depression and anxiety, antidepressants and anti-anxiety drugs can be used for treatment while treating prostatitis.
