Male pyelonephritis is a type of upper urinary tract infection, which is relatively common. Usually, the early symptoms of any disease are not very obvious. 1. Acute pyelonephritis

(1) General symptoms include high fever and chills, with body temperature usually between 38 and 39°C, but can be as high as 40°C. Fever patterns vary, generally presenting as a remittent type, but can also be intermittent or prolonged. Accompanied by headaches, body aches, and profuse sweating when the fever subsides.

(2) Urinary system symptoms Patients have low back pain, which is usually dull or sore, with varying degrees of severity. A few have abdominal cramps that radiate along the ureter toward the bladder. During physical examination, there is tenderness at the upper ureteral point (the intersection of the outer edge of the rectus abdominis and the umbilical cord) or the costal waist point (the intersection of the outer edge of the psoas major and the twelfth rib), and positive renal percussion pain. (3) Gastrointestinal symptoms may include loss of appetite, nausea, vomiting, and some patients may experience pain in the upper or middle abdomen.

2. Chronic pyelonephritis

The clinical manifestations of chronic pyelonephritis are complex and prone to recurrent attacks. The symptoms are milder than those in the acute phase and may sometimes present as asymptomatic urine. More than half of the patients have a history of acute pyelonephritis, followed by symptoms such as fatigue, low fever, anorexia, and backache, accompanied by lower urinary tract irritation symptoms such as frequent urination, urgency, and pain. Acute attacks also occur from time to time. In the past, patients with a course of more than half a year or one year were considered chronic pyelonephritis. In recent years, it has been proposed that chronic pyelonephritis is only considered chronic when there is scarring of the pelvis, pyelonephrography shows deformation of the pelvis, hydrops, uneven kidney shape, or unequal sizes of the two kidneys. There may be damage to renal tubular function, such as decreased concentrating function, hypoosmotic and low specific gravity urine, increased nocturia and renal tubular acidosis. In the late stage, there may be damage to glomerular function, azotemia and even uremia. Renal hypertension is often caused by chronic pyelitis. It is generally believed that the patient's hypernephritis and the release of some vasoconstrictive polypeptides are related to vascular sclerosis, stenosis and other lesions. A small number of patients have their hypertension improved after removing one of the diseased kidneys.
