Prostatic hyperplasia, formerly known as prostatic hypertrophy, is one of the common diseases in elderly men and a benign lesion of the prostate. The cause of the disease is related to the imbalance of androgens and estrogens in the human body. The lesion originates from the glandular tissue, connective tissue and smooth muscle tissue of the middle lobe or lateral lobe under the mucosa of the posterior urethra, forming mixed spherical nodules. The hyperplasia of the lateral and middle lobes is obvious, protruding into the bladder or urethra, compressing the bladder neck or urethra, causing lower urinary tract obstruction. When prostatic hyperplasia causes obstruction, the bladder detrusor thickens, and trabeculae, cavities and diverticula appear in the mucosa. Long-term dysuria causes the bladder to be highly dilated, the bladder wall to become thinner, the intra-bladder pressure to increase, and the end of the ureter loses its valve function, resulting in vesicoureteral reflux.

Symptoms and signs

1. Frequent urination Frequent urination is the earliest manifestation, first at night, and then during the day. In the later stage, the residual urine increases after the bladder detrusor fails to compensate, and the effective bladder capacity decreases, which also makes the frequent urination more serious.

2. Dysuria Progressive dysuria is a notable feature of this disease, manifested by delayed onset of urination, thinning of the urine stream, shortened range, and dripping after urination.

3. Hematuria The capillaries on the prostatic mucosa are congested and the small blood vessels are dilated, and they are pulled by the filling and contraction of the bladder and rupture and bleed. Hematuria may also occur when combined with bladder tumors.

4. International Prostate Symptom Score (IPSS) Patients are asked 7 questions about urination, and each question is scored according to the severity of the symptoms (0-5 points), with a total score of 0-35 points (no symptoms to very severe symptoms). Among them, 0-7 points are mild symptoms; 8-19 points are moderate symptoms; 20-35 points are severe symptoms. Although the IPSS analysis attempts to quantify the degree of symptom change, it is still affected by subjective factors.

Physical examination: In case of acute urinary retention, the lower abdomen is swollen. A full bladder can be felt in the suprapubic area. Rectal examination shows that the prostate is enlarged, smooth, elastic, and the central groove becomes shallow or disappears. Prostatic hyperplasia can be divided into 3 degrees according to the degree of glandular enlargement. Grade I enlargement: The prostate is 1.5 to 2 times larger than normal, the central groove becomes shallow, and the distance of protrusion into the rectum is 1 to 2 cm; Grade II enlargement: The gland is moderately enlarged, 2 to 3 times larger than normal, the central groove disappears or slightly protrudes, and protrudes into the rectum by 2 to 3 cm; Grade III enlargement: The gland is severely enlarged, protruding into the rectum by more than 3 cm, the central groove is obviously prominent, and the upper edge cannot be touched by the fingers during examination (Table 1).
