The corpus cavernosum is a part of the penis that is relatively sensitive, mainly distributed in the glans penis. By stimulating the glans penis, the penis can become erect, which is related to the sensitive corpus cavernosum. If the corpus cavernosum tunica albuginea ruptures, it is caused by excessive sexual intercourse, and too much twisting of the penis can also cause the tunica albuginea to rupture, which can lead to erectile dysfunction and affect future fertility.

This situation is not uncommon in clinical practice. It usually occurs during intense sexual intercourse, especially when the woman is on top or in more unusual sexual positions. It can cause the penis to twist or be suddenly hit, leading to tearing of the frenulum or even rupture of the tunica albuginea or corpus cavernosum.

When the penis is erect, a large amount of blood will be injected into the corpus cavernosum of the penis, and the volume of the corpus cavernosum will increase suddenly, making the tunica albuginea wrapped outside it thinner and in a highly tense state, and the brittleness will also increase significantly, just like a small balloon that is blown up. At this time, if it is suddenly affected by external force, the expansion and elasticity of the tunica albuginea will exceed the limit, and the tunica albuginea and corpus cavernosum on one side are prone to rupture. Generally, a "click" sound can be heard. After the injury, the penis is severely injured locally, the penis quickly becomes soft, and swelling and congestion appear immediately. The penis appears bent and deformed, and may be biased to the opposite side, swollen in an "S" shape, or drooping downward. If the penile fascia is intact, the hematoma is limited to the penis. If the penile fascia is ruptured, the penile hematoma will spread to the scrotum and perineum.

The penis may compress the urethra due to blood swelling, causing dysuria, but the urethra is generally not damaged. If accompanied, there will be blood in the urine or bleeding from the urethra. Local palpation can feel a firm, swollen, isolated mass at the penile fracture site, and the penis skin can slide back and forth on it.
