Scrotal angiokeratoma is also known as Fordyce angiokeratoma. It mainly occurs in the scrotum of middle-aged and elderly people, and occasionally in the labia. It often increases with age. There are multiple dome-shaped papules in the scrotum, with a diameter of 1 to 4 mm. In the early stage, the lesions are bright red, soft, and can shrink when pressed. In the late stage, they are dark red or purple, hard, and have mild wart-like hyperplasia.

Often arranged in a linear pattern along the superficial veins or scrotal skin lines, the surface of the lesion is usually smooth, sometimes slightly flaky, and can fade with pressure. Sometimes the lesion occurs on the penis or glans, and rarely on the calf, thigh, and bulbar conjunctiva. Generally, there is no obvious discomfort, but there is occasionally a mild itching sensation. It is easy to bleed after the injury. It is often accompanied by epididymal tumors, hernias, varicocele, oral mucosal varicocele, and scrotal elastic fiber defects.
