Physical Examination: A careful physical examination is crucial for the early diagnosis of acute scrotal disease. In the early stage of testicular torsion, there may be no redness or swelling of the scrotum, but redness and swelling of the scrotum may be seen if the torsion lasts for more than 12 hours. The testicles are obviously swollen and tender. In patients with a longer torsion time, the boundary between the testicle and the epididymis cannot be palpated due to severe local swelling. The aggravation of scrotal pain when the patient is lifted, i.e., positive Prehn's sign, is a major feature of this disease. Due to spasm of the levator testis muscle and torsion and shortening of the spermatic cord, upward displacement of the testicle or a molded position and anterior epididymis are important manifestations of testicular torsion. Epididymitis usually occurs in adults. The affected side of the scrotum is enlarged, the skin is red and swollen, the spermatic cord is thickened and tender, and the enlarged and painful testicle and epididymis can be felt more clearly. The testicle on the affected side often droops, and Prehn's sign is negative. When there is sudden severe pain in the scrotum, the skin is red, swollen, shiny, and hard, with erythema and blisters, followed by moisture, purple-black and necrosis, gas accumulation in the diseased tissue, and a twisting sound when touched, these are all characteristics of scrotal gangrene. |
