Congenital azoospermia is also known as Klinefelter syndrome. It is a disease caused by abnormal sex chromosomes. During the process of human egg or sperm, it must undergo meiosis. If the sex chromosomes of sperm and oocytes are not separated during meiosis, the zygote formed after fertilization will have an extra X chromosome, forming XXY. This extra X chromosome can come from the egg or sperm. In addition, if the X chromosome does not separate during the cleavage process of the fertilized egg, an extra X chromosome may also appear. Symptoms of patients with congenital azoospermia: small and hard testicles. Histological examination shows fibrosis and hyalinization of the testicular seminiferous tubules, occlusion of the tubular lumen, azoospermia, proliferation or aggregation of interstitial cells with poor function, slow testosterone production, low blood testosterone concentration, and low response to exogenous gonadotropin stimulation. However, the patient's plasma and urine luteinizing hormone and follicle-stimulating hormone are elevated, and the high secretion of luteinizing hormone will stimulate the testicular interstitial cells, increase estradiol, and increase the estradiol/testosterone ratio. As a result, congenital azoospermia causes the patient's breasts to develop into female-type breasts.

The incidence of congenital azoospermia is 0.1%. Before puberty, there are generally no obvious symptoms. During puberty, symptoms gradually become obvious, with poor development of the vulva, short penis, small and hard testicles, delayed or even no secondary sexual development in males, fine and pale skin, poor development of body hair, sparse pubic hair and beard, and often no armpit hair, unclear Adam's apple, tall stature, and lower body longer than upper body. Nearly half of patients have female-type breasts. Congenital azoospermia patients have low libido and poor sexual function. Except for a few patients, almost all of them are azoospermic and infertile.
