The incidence of this disease is very low, and the problem of male scrotal swelling is also more serious. What causes male scrotal swelling? Scrotal enlargement (oscheocele) refers to the scrotal wall or vaginal testicles, epididymis and spermatic cord, etc., which are pathologically swollen due to acute or chronic inflammation, parasitic invasion, organic changes, tumors, etc., or increased inflammatory exudation, resulting in edema and effusion. If the peritoneal processus vaginalis does not close or does not close completely after birth, the contents of the abdominal cavity can reach the scrotum. Treatment should find the primary disease of scrotal enlargement and treat the cause. Check 1. Physical examination (1) Local palpation Local palpation is the most important and simple method for diagnosing scrotal enlargement. The scrotum and its contents belong to the external reproductive organs. The examiner must be proficient in the gross anatomy of the scrotal contents in order to clearly diagnose whether the lesion occurs in the scrotum or the scrotal skin; whether it originates from the scrotal contents or comes from the inguinal region; whether it is testicular enlargement or epididymal enlargement; whether it is cystic enlargement or solid enlargement. During the examination, the patient should first take a standing position, and then do a supine examination. The examiner should palpate with both hands at the same time to facilitate left-right comparison. (2) Cough shock: For swellings originating from the inguinal area, cough shock examination can be used to distinguish inguinal effusion from hydrocele. (3) The light transmission test is of great value in distinguishing whether the scrotal enlargement is cystic or solid. (4) Scrotal puncture: To identify the nature of scrotal effusion, fluid can be drawn for examination. However, for substantial swelling, tissue can also be aspirated for pathological examination. (5) Other examinations include prostate examination, abdominal examination and related systemic examinations. 2. Laboratory inspection Routine blood and urine tests and erythrocyte sedimentation rate tests have certain auxiliary diagnostic significance in acute and chronic infections. Finding microfilariae under a blood smear microscope is of diagnostic value for scrotal elephantiasis. Determination of human chorionic gonadotropin (HCG) and alpha-fetoprotein (AFG) in blood or urine is of great significance for the diagnosis of testicular chorionic epithelial carcinoma or embryonal carcinoma. 3.
